Syrokomli 11/8 (30-102 Kraków) ["Stella Virium Foundation"] which is the administrator of personal data within the meaning of the provisions of the Act of August 29, 1997 on the protection of personal data (Journal of Laws of 2002, No. 101, item 926, as amended)), the purpose of processing is the implementation of the statutory objectives of the Stella Virium Foundation, including in particular those related to the implementation and promotion of the Helping Hand project. Each person has the right to access and correct the data collected about him, the right to submit a written, motivated request to stop processing his personal data due to his special situation, as well as the right to object to the processing of his data in special cases under the Act about personal data protection. I acknowledge that my personal data is processed by the Stella Virium Foundation for the statutory purposes of the Stella Virium Foundation, including in particular related to the implementation and promotion of the Helping Hand Project and may be made available only to entities cooperating with the Stella Virium Foundation in the implementation of the Project, and also, that I am entitled to access and correct my data, and I provide my personal data voluntarily.

I declare that I authorize the STELLA VIRIUM Qualifications Development Foundation with its seat in Krakow, ul. W. Syrokomli 11/8 (30-102 Krakow) ["Stella Virium Foundation"] to use my image in the activities of the foundation in connection with the implementation and promotion of the Helping Hand Project, and in particular to fix, reproduce and market recordings or photos containing my image in connection with the implementation and promotion of the Helping Hand II project and to disclose my name and surname in connection with the participation in the Helping Hand II Project in all announcements, announcements and information about the Helping Hand II Project and its results, including publishing my personal data in other media that are authorized by the Organizer - Stella Virium Foundation - Partners of the Helping Hand II Project.
